Chronic Disease Management (CDM)
The Medicare referral allows people with chronic disease who are being managed by their general practitioner (GP) under a Chronic Disease Management (CDM) plan to access Medicare rebates for Exercise Physiology services.MA chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions. However, the CDM items are designed for patients who require a structured approach and to enable GPs to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team.
In order for you to be eligible to receive a Medicare rebate you must be referred from your GP with the following items:
- - GP Management Plan GPMP – Item 721 or 732 (review)
- Team Care Arrangements TCAs Item 723 or 732 (review)
- This program was previously known as the ‘Enhanced Primary Care (EPC)’ program.
Please note, your general practitioner will determine whether you are eligible to be referred on the EPC plan.
